I understand that there are risks injury associated with physical activity.   I hereby agree to release Dance Dynamics (DD) and hold DD harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation.  I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will. I hereby agree to indemnify and hold harmless DD and any of their officers, agents and employees from any liability or claim or action for damages resulting from or in any way arising out of the participation in the program by the person registered.  I give DD my full and unlimited permission to use my child’s picture, image, likeness or video (including retouching, editing, cropping or any other alteration) in or on any form of advertising without compensation.  These images may be used in (but not limited to) publications such as emails, brochures, newsletters, magazines, website and videos.  I waive any right to inspect or approve these products.  DD does not administer medication and any medication is the sole responsibility of the individual’s parents or guardian. DD prefers all dancers to stay in studio during breaks and asks that if a dancer leaves, they have parent/guardian permission and are wearing appropriate attire (no leotards, midriff tops or booty shorts). DD may take children outside and throughout the Downtown Summerlin mall.
In addition to phone calls and emails, DD uses text as a method of communication.  Should you not want DD to text you, please contact the front desk to opt out.Covid-19 and Disease Waiver: Participants understand that attending in person dance classes at Dance Dynamics is of their own free will.   Participants will abide by all social distancing and sanitation requirements required by DD.  Participants will not come into the studio if anyone in the household is sick or has a symptom or a suspected symptom of Covid-19.  Nor will participant come on premise for a minimum of 14 days if participants has traveled from areas on the CDC Level 3 Travel Health Notice. DD has implemented recommended protocols and preventative measures recommended by the CDC to help reduce the spread of Covid-19 however DD cannot guarantee that participants will not become infected with Covid-19.  By signing this agreement, participant acknowledge the contagious nature of Covid-19 and other diseases and voluntarily assume the risk that participant may be exposed or infected by attending classes at DD.  Such exposure or infection may result in personal injury, illness, permanent disability and death.  Participant understands that the risk of becoming exposed to or infected by Covid-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, DD instructors, volunteers, and program participants and their families. Participant voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to participant (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that participant may experience or incur in connection with attendance at DD or DD events.  Participant hereby releases, covenant not to sue, discharge, and hold harmless DD, its instructors, agents, owners, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. Participant understands and agrees that this release includes any claims based on the actions, omissions, or negligence of DD, its instructors, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any program with DD.
The undersigned acknowledges and agrees to comply with DD's procedures at all times. The participant has read and understood the terms of this assumption of risk, release and waiver of liability, and indemnity agreement and agree to its terms.
